OneTide Wellness Invoice Payment Terms & Conditions

Welcome to OneTide Wellness. These Terms and Conditions ("Terms") govern the payment of invoices for medical services and prescriptions provided by OneTide Wellness ("we," "our," "us").

1. Acceptance of Terms

By paying an invoice, you ("patient," "you," "your") agree to be bound by these Terms. If you do not agree with these Terms, please do not proceed with the payment.

2. Payment Methods

We accept the following payment methods:

  • Credit/Debit Card

  • Bank Transfer

  • Online Payment Platforms (e.g., Stripe)

  • Insurance Payments (where applicable)

3. Invoice Due Date

Invoices are due upon receipt unless otherwise stated. Payment must be made within 30 days from the invoice date. Failure to pay within this period may result in additional charges and potential disruption of services.

4. Late Payments

If payment is not received by the due date, we reserve the right to:

  • Charge a late fee of [insert amount or percentage] per month on the outstanding balance.

  • Suspend or terminate services until the outstanding balance is paid in full.

  • Seek legal action to recover the outstanding amount, including any legal fees incurred.

5. Insurance Claims

If your insurance policy covers our services, you are responsible for ensuring that we receive the correct information to process claims. Any balance not covered by insurance is your responsibility.

6. Refunds and Disputes

  • Refunds: Payments made are generally non-refundable. Exceptions may be made in cases of billing errors or services not rendered. Refund requests must be submitted in writing within 14 days of the invoice date.

  • Disputes: Any disputes regarding the invoice must be reported to us in writing within 14 days of the invoice date. We will review and address disputes on a case-by-case basis.

7. Confidentiality

We are committed to protecting your privacy and maintaining the confidentiality of your medical and personal information in accordance with applicable laws and regulations.

8. Amendments

We reserve the right to amend these Terms at any time. Any changes will be effective immediately upon posting the updated Terms on our website or through other communication channels.

9. Governing Law

These Terms are governed by and construed in accordance with the laws of Arizona, USA. Any disputes arising from these Terms will be resolved in the courts of Arizona, USA.
